Blue Form

Today my colleague called me and said she had collected the blue form for me from the HR. My major nightmare is started because this is the form to perform my medical check up for the conversion.

I had experience on medical check up during my first job but don't know why this time quite nervous. Perhaps just as what I had mentioned earlier, I had did some sinful job last Friday, hope it won’t affect my result. MM.
